Evolve Performance is a fee-for-service business which means we do not actively participate with insurance companies. We provide all of our clients with a superbill including diagnosis codes and procedure codes. We encourage all patients to inquire about their out of network insurance benefits prior to starting treatment.
Payment is due at the time of service and we accept HSA, credit card and Venmo. The initial evaluation fee is $200, all follow up appointments are $160.
This self pay model of treatment allows us to provide 1:1 individualized care to the entire body without insurance restrictions. We are looking forward to you investing in your health with us!
